For each person you refer to our practice you
will receive a special reward. The more patients
you send, the larger the reward.

$25 credit on account to be used towards
future work per referred patient. |
| 6-10 Referred Patients: |

referred patient. |

| 21+ Referred Patients: |
$100 American Express Gift Card per referred
patient, plus 10% off all out of pocket expenses
within practice. |
*Reward is not given until
referred patient comes in for their initial
visit and begins treatment.
